Introduction: Repeatable neuropsychological tests are a relevant clinical demand for longitudinal diagnosis. When it comes to verbal memory tests, equivalence studies of alternate list forms are paramount to ensure psychometric validity among forms in terms of difficulty and psycholinguistic characteristics. However, no study up to date conducted a formal analysis of inter-form familiarity equivalence among alternate forms of the Free and Cued Selective Reminding Test (FCSRT). Objective: Thus, the present study aims to present the cross-cultural adaptation to Brazilian Portuguese, familiarity and performance equivalence of three alternate list versions of the FCSRT in healthy individuals. Methods: After conducting translation and back translation of FCSRT semantic categories, the authors performed a group verbal fluency task with words associated with each of the 16 semantic. An exclusion criterion was established in term of prototypicality and word length. Later, an equivalence study was initiated divided into four phases with four control groups. Phase 1 consisted of an initial exploratory study in which a group of 25 college students judged the subjective feeling of familiarity of a list of 313 words. Highly familiar words were excluded. In phases 2 (n = 56) and 3 (n = 76), a Familiarity Questionnaire was presented containing all preserved words from the previous phase. The final words were chosen according to familiarity scores, and three versions of the FCSRT were developed. A repeatable measures analysis was conducted in order to verify inter-form familiarity equivalence between forms. In phase 4, the newly developed versions were administered in three separate groups of healthy individuals (n=132). A MANOVA analysis was performed controlling education and age effects in order to observe inter-form performance equivalence between forms. Results: Analysis revealed no significant familiarity or performance differences between the three 16-word list versions of the FCSRT, indicating overall inter-form equivalence. Conclusions: The three developed alternate versions of the FCSRT are equivalent. Limitations and recommendation regarding equivalence studies are discussed.
