[pt] Este trabalho analisa as diferentes formas de inserção do saber psicanalítico no contexto institucional, notadamente nos CAPS da Rede de Serviços Substitutivos em Saúde Mental. Procura resgatar a forma de constituição destes dispositivos e equipamentos que tentam levar adiante a proposta da Reforma Psiquiátrica brasileira. Estabelece alguns pontos fundamentais da dinâmica clínica e social no cuidado e na assistência em saúde mental, com vistas a melhor demarcar os desafios da clínica ampliada neste contexto. Serve-se das experiências clínicas empreendidas por profissionais em todo o Brasil para pensar um modelo de psicoterapia de grupo que possa fazer frente às necessidades que a instituição apresenta, e às demandas dos chamados usuários do sistema público de saúde. Retoma o desenvolvimento das técnicas grupais em psicanálise e defende a grupoterapia como dispositivo clínico e técnico, que pode apresentar bons resultados, tanto no plano da reabilitação social dos pacientes, quanto no âmbito da clínica da subjetividade.

[en] This paper explores the different methods for introducing psychoanalytic knowledge into the modern institutional care system, with focus on the CAPS Network substitutive services in Mental Health. It seeks to define and evaluate the origns and evolution of these practices and facilities that aim to implament the proposed Psychiatric Reform in Brazil. Some of the fundamental aspects and social dynamics of clinical assistance and mental health care are established in order to better define the challenges of extended clinic in this context. This research considers clinical trials undertaken by professionals in Brazil to develop a model of group psychotherapy able to address the needs and requirements inherent in the current institutional public health system, while meeting clinical demands and providing individual treatment plans for the users/patients. The modern development of group techniques in psychoanalysis is discussed. In addition, group therapy - as a clinical and technical device capable of delivering positive results - is supported both in terms of the social rehabilitation of patients, as well as in the the subjective clinic.
